Consent to Share Personal Information with the Union

Dear AMFA Member:
Upon the implementation of the payroll dues deduction process at WestJet, the carrier requires consent from all AMFA-WestJet Members to share personal information with the Association.
The personal information includes but is not limited to, contact information, job status details, and leave of absence details (leave type and dates) as defined in the collective agreement (detailed medical data will not be provided).
This information is used to determine the rights of membership and conduct union business.
IMPORTANT! Each member must complete the Consent to Share Personal Information with the Union Form by October 11, 2024. After you submit the form electronically, you must check your email to verify your identity and consent form.
